What is patellofemoral pain?

Patellofemoral pain syndrome (PFPS) is pain arising from the patellofemoral joint - the articulation between the kneecap (patella) and the groove on the front of the femur (thigh bone) through which it tracks during knee movement. It is the most common cause of anterior (front-of-knee) pain in active people, affecting runners, cyclists, hikers, court sport players, and anyone who performs repeated squatting or stair climbing.

The pain arises when the kneecap does not track smoothly through its groove during movement, creating excessive pressure and friction on the cartilage and soft tissue structures on its underside. This mismatch in patellar tracking is most commonly driven by weakness in the quadriceps (particularly the VMO - vastus medialis oblique) and hip abductors, which fail to control the knee alignment during loading, and by tightness in the IT band and lateral retinaculum, which pull the kneecap laterally.
