What is lower back pain?

Lower back pain (LBP) is the most common musculoskeletal condition globally - affecting around 80% of adults at some point in their lives - and the leading cause of disability worldwide. The vast majority of acute lower back pain (lasting less than 6 weeks) has no identifiable structural cause: it is non-specific lower back pain, driven by muscle guarding, joint stiffness, and the self-perpetuating cycle of pain leading to reduced movement leading to more pain.

The lumbar spine consists of five vertebrae (L1-L5) separated by intervertebral discs, supported by a complex network of ligaments, muscles, and fascia. The most common sources of lower back pain include muscle and ligament strain, facet joint irritation, and disc-related issues. In the majority of cases, non-specific LBP responds well to conservative management: staying active, reducing fear-avoidance behaviours, and targeted exercise.

Chronic lower back pain (persisting beyond 12 weeks) is more complex, often involving a combination of physical, psychological, and lifestyle factors. The evidence consistently shows that staying active and avoiding prolonged rest is more effective than immobilisation, even in painful episodes.
