What causes calf swelling?

Calf swelling is a common complaint across a wide range of active and sedentary populations. It can result from localised muscle injury, overuse, prolonged standing or sitting, or circulatory causes, and the appropriate management differs significantly depending on the underlying cause.

In active and sporting populations, the most common causes are calf muscle strains (partial or complete tears of the gastrocnemius or soleus), the delayed-onset muscle soreness and micro-inflammatory swelling that follows hard training sessions, and Achilles tendinopathy, which can produce localised swelling in the tendon and surrounding tissue. Prolonged running on hard surfaces, uphill running, and sudden increases in training volume all predispose the calf to these conditions.

In less active populations and during prolonged sedentary periods (long-haul travel, desk work, post-operative recovery), calf swelling is often related to dependent oedema - fluid pooling in the lower leg due to reduced circulatory return from the calf pump mechanism that normally pushes blood back towards the heart during walking. This is typically bilateral and worsens throughout the day.

Important note: calf swelling that is unilateral (one leg only), accompanied by warmth, redness, and pain that is worse when the foot is dorsiflexed (pointing toes up), should be assessed medically without delay - these are the cardinal signs of a deep vein thrombosis (DVT), which requires prompt treatment. If in any doubt, seek medical advice before using compression.

Understanding calf swelling

Icon for verifiedSymptoms to recognise
  • Visible puffiness or enlargement of the calf, either diffuse or localised to the muscle or Achilles region
  • A feeling of heaviness, tightness, or pressure in the lower leg
  • Tenderness when pressing on the swollen area, with the degree of tenderness reflecting the underlying cause
  • Stiffness in the calf and ankle, particularly first thing in the morning or after periods of inactivity
  • Reduced ankle dorsiflexion (ability to pull the foot up) when swelling is significant
  • Worsening swelling throughout the day with prolonged standing or sitting, improving with elevation overnight

Graduated compression is the gold standard non-pharmacological intervention for lower leg swelling. The Bearhug calf support applies graduated pressure that is greatest at the ankle and progressively reduces up the leg - mimicking and augmenting the calf's own pumping mechanism to move fluid from the lower leg back into the circulatory system.

For sports-related calf swelling - from muscle strains, overuse, or Achilles issues - compression reduces the inflammatory fluid at the site of tissue damage, limits further fluid accumulation during activity, and accelerates the clearance of metabolic waste products from hard training sessions. This reduces both pain and recovery time, allowing a faster return to training.

For non-specific calf swelling related to prolonged standing or sedentary periods, wearing calf sleeves throughout the day maintains the circulatory return that prevents fluid pooling. Bearhug's bamboo calf sleeves are comfortable enough for all-day wear under clothing, lightweight, and sufficiently discreet to wear in an office or work environment.

The bamboo fabric's thermoregulating properties ensure the calf stays comfortable during extended wear - neither overheating during activity nor losing its warmth benefit during sedentary periods.
